FOR ALL ATTENDING CAMP listed below are the forms that we need to have for processing BEFORE JUNE 11^TH.

1. Medical Form 34605 (643 KB PDF file) – anyone that attends day camp must fill out section A & C.  This is required for Cub Scouts, Tot Lots youth, Adults, and den chiefs

2. BB-Archery Consent (90 KB PDF file) – required for any youth that will shoot BB and archery.  This is required for Cub Scouts and den chiefs.

3. Staff Agreement (72 KB PDF file) – required for every adult that volunteers at camp

4. Den Chief Agreement (65 KB PDF file) – required for every den chief that volunteers at camp

5. Adult Volunteer / Den Chief interest application (55 KB PDF file) – required for every adult AND every den chief  (for den chiefs you must have your SM approval signature as well)

6. Authorization to pick up (65 KB PDF file) – required for any Cub Scout and/or Den Chief that is having someone that is not their parent or guardian pick them up

7. Rock Wall Release (66 KB PDF file) – we still have not secured the rock wall yet, but we will need the release signed for everyone that will climb the rock wall.
